The Prevention Plus Plan is an outpatient wellness program ideally suited for individuals and
families who are uninsured, underinsured, have high deductibles, or have a pre-existing condition that is not covered
by insurance. This pay-one-price wellness plan fills the gaps, insuring that members receive the most complete and
sophisticated preventive care available, with all diagnostic and laboratory testing performed on-site.
For one annual fee, you will receive a thorough physical examination, including a medical
history, complete blood workup, urinalysis, as well as all other recommended diagnostic and screening tests. Your
Elite Health physician will then detailing your current health status and specifying recommendations for improving
your well being, including nutritional advice based on the Reality Diet program. A small co-payment is required
for follow-up visits, either with your primary care physician or with any of our on-staff specialists. |
